Ocena wpływu nieprawidłowego profilu biofizycznego płodu na występowanie krwawienia dokomorowego (IVH) i zamartwicy u noworodka

Summary
Introduction. A supervion of foetus by biophysical profile is a standard procedure. It contributes to early diagnosis of asphyxia which IVH follows. Intraventricular Hemorrhages I Grade do not cause complication, Grade II evokes ventricular system ectasy, Grade III and IV causes hemorrhagic hydrocephaly. A correct CTG record informs about foetus’ wellbeing, the lack of intrauterine anoxia and asphyxia danger. A fluid volume estimation is required component of ultrasonography and its disorders are symptoms of pathology. An estimation of foetus wellbeing by ultrasonography mainly consists of doppler vessel flow in middle cerebral artery and umbilical artery.
Aim. The aim of this project is to demonstrate correlation between incorrect biophysical profile of a foetus and IntraVenticular Haemorrhage and asphyxiation prevalence in infant.
Material and methods. The material includes analysis of live births from 1st January 2006 to 31st December 2008 in Gynaecological/Obstetric Clinic of CZMP Institute in Łódź, analysis of women’s giving birth at least in 30 week, selection of 1000 infants and their 934 mothers, so the group of 1934 people under investigation recived. Statistical calculations – test chi? and the Pearson product-moment correlation coefficient.
Results. Among 1000 newborns 141 cases of asphyxia and 143 of IVH determined, in ultrasonography, around the third day of age.
Conclusion. 1. In the group of examined infants the statistical correlation between asphyxiation, Intraventricular Hemorrhage and incorrect NST records (Rp = ~22%), decelerations (Rp = ~15%) and oscilation disorders (Rp = ~14%) in foetus has been shown. 2. The occurrence of asphyxiation and IVH did not show statistical correlation with incorrect amniotic fluid volume (Rp = ~5%) and incorrect vessel flow (Rp = ~9%). 3. The analysis showed the correlation between infant’s IVH and its bad condition after delivery and incorrect biophysical profile. The strenght of correlation Rp = ~0.3712 = ~37% is moderate (statisticaly significant).
