Summary
Lower respiratory tract infection can impact on the pathogenesis and the clinical course of COPD in several ways. These include the following:
1. Chronic colonization/infection of the lower respiratory tract by bacterial pathogenes induces chronic inflammatory response with lung damage („vicious circle hypothesis”)
2. Childhood lower respiratory tract infection impairs lung growth reflected in lower FEV1 in adulthood
3. Chronic infection of respiratory tissues makes the patient more sensitive to tobacco smoke or acts synergistically with it
4. Microbial antigens present in the lower airways induce hypertensitivity that enhances airway hyperreactivity.
This article summarizes current knowledge about first three mechanisms by which infection can influence COPD pathogenesis.
