Summary
Wolff, Parkinson and White described electrocardiographic criteria of the syndrome in 1930, although they were not aware of the pathology underlying this condition. Since then a great progress has been made in understanding of the anatomical, pathophysiological and finally electrophysiological basics of this disorder. The authors present current state of knowledge regarding epidemiology, presentation and treatment of children with WPW syndrome. Patients suffering from preexcitation syndrome are at risk of developing episodes of atrioventricular reentry tachycardia but also a sudden cardiac death. SCD may be the first symptom of the disease therefore it is crucial to stratify the risk in asymptomatic patients. Risk stratification can be made using noninvasive tools, like Holter ECG and stress test or with invasive electrophysiological study. In symptomatic patients, antiarrhythmic drug prophylaxis could be used, but since ablation procedure is the only way to remove the cause of the disorder it has become a treatment of choice in recent years. In children with body mass above 15 kg the effectiveness and safety of the ablation is very high and comparable to the results achieved in the adult population.
