Summary
Epidemiological data on chronic diseases indicate the need for enhanced preventive actions at all stages of prophylaxis – directed both at early detection and prevention of complications in people already ill. General practitioner (GP) and occupational medicine physician are those doctors who most regularly have contact with patients not aware of any health problems who can be implemented with the preventive actions. Those patients are examined due to administrative reasons to obtain the medical certificate, so the circumstances are completely different from visits to GP. In case of those two medical specialities, the need to launch the system of information exchange and permanent cooperation becomes increasingly clear. Detected and treated by general practitioner illness can constitute an obstacle to carrying out the work while occupational doctor has unique chance to detect early the deviations of health condition, which can require further diagnostic and medical treatment in primary care centre. In the publication an identification of strengths and weaknesses of prevention tackled by occupational medicine and primary care was made as well as the chance of improving the effectiveness of these actions by formalising the exchange of information as a form of improving mutual cooperation was pointed. To achieve this aim, the models of medical certifications have been developed. They comprise the individual and external documentation issued alternately by an occupational doctor to primary care or by a general practitioner to occupational medicine.
