Czy skleroterapia guzków krwawniczych jest tylko metodą historyczną?

Summary
Hemorrhoidal disease, besides an anal fissure, is one of two most frequent inflammatory proctologic diseases. The way of treatment of hemorrhoidal disease mainly depends on the degree of the clinical progress of illness, as well as on age, general condition of the patient, and on psychological attitude of patient to the proposed method of treatment. Sclerotherapy is one of the oldest methods of treatment of hemorrhoids. The first doctor who applied obliteration of hemorrhoids in the form of iron persulfate injection was Morgan from Dublin in 1869. Principle of sclerotherapy consists in injection of obliterating agent causing local inflammatory condition and the secondary fibrosis and adhesion of mucous membrane to the muscular layer below. Various obliterating agents are used, the most frequently polidocanol 3%. Sclerotherapy is indicated in moderately advanced period of hemorrhoidal disease: II, more rarely III degree, whereas the primary contraindication is allergy to the obliterating agent. The crucial role in the effectiveness of therapy plays correct qualification of patient to the obliteration and its proper technical execution. Too shallow injection of the substance can be ineffective, whereas too deep injection can lead to formation of anal abscess. Conclusions: Sclerotherapy, although it is the historical method still has its place in treatment of moderately advanced hemorrhoidal disease. Further development of the method can consist in use of different, new obliterating preparations.
