Rola diety w reumatoidalnym zapaleniu stawów – przegląd systematyczny badań

Summary
Introduction. Rheumatoid arthritis (RA) is a chronic, systemic connective tissue disease of the immune and unknown etiology. The characteristic symptoms of RA include symmetric joint pain and swelling of hands and feet, as well as morning stiffness. Except the pharmacotherapy important role in the treatment of patients with RA plays diet.
Aim. Evaluation of clinical efficacy and safety of diets to improve the quality of patient’s life with RA.
Material and methods. Evaluation of clinical efficacy based on the results of primary studies and secondary evidence found in the context of a systematic review. The following literature databases were searched systematically: Medline, Embase, Cochrane Library and others.
Results. Indomethacin vs. indomethacin + omega-3: indomethacin reduces the DAS28 by 34.2%; indomethacin + omega-3 reduces the DAS28 by 41.9%.
Mediterranean diet (MD) vs. standard diet: at 12 week of study in patients with the MD group has been reported improvement in 3 of 4 variables compared to baseline (including disease activity measured in DAS28 and HAQ scale, subjective assessment of health status by the SF-36).
Omega-3 vs. placebo: in most studies has been shown the benefits of taking omega-3, including reduction: the duration of morning stiffness, number of tender and swollen joints, pain of arthritis.
Vegetarian/vegan/mediterranean diet vs. standard diet: vegetarian and mediterranean diet significantly reduced pain compared to the standard diet.
Conclusions. Mediterranean diet significantly reduces disease activity in patients with active RA. Scientific evidence indicates the beneficial effects of omega-3 in RA, observed in a reduction: the number of swollen, tender and painful joints, morning stiffness, joint pain, disease activity and the need for NSAIDs.
