Summary
Introduction. Subglottic stenosis, as a complication of prolonged endotracheal intubation, is a current problem in pediatric laryngology because of growing population of prematurely born children.
Aim. The aim of our study was to make an assessment of the cases in terms of frequency and severity of the complications.
Material and methods. The study was performed on a group of 92 children hospitalized for postintubation subglottic stenosis. Two groups of term and preterm newborns were analyzed for total intubation time, subglottic stenosis grade, influence of a birth weight on a stenosis grade and a role of accompanying diseases.
Results. In our group 55% of children were premature newborns. 94% among them had additional systemic diseases that considerably increased the risk of postintubation complications and subglottic stenosis. In our study, it was noted that premature newborns were two times longer intubated than other newborns, 54 days on average. In that group of children a significantly more patient had high grade laryngeal stenosis (III and IV Cotton’s grading system), especially if it was corelated to a lower birth weight.
Conclusions. Premature newborns with low birth weight are particularly prone to develop a respiratory disorder associated with subglottical stenosis. Medical science development increases a chance of survival in that group of children. Therefore, the patients of low birth weight with a high grade laryngeal stenosis start to become a significant problem in pediatric laryngology.
