Zastosowanie preparatów zawierających chlorheksydynę u pacjentów obciążonych chorobami ogólnoustrojowymi z zapaleniami protetycznymi błony śluzowej jamy ustnej

Summary
Introduction. Patients with systemic diseases, such as diabetes mellitus, hypertension, digestive system disorders, rheumatoid arthritis, who refer to the dentist and use acrylic dentures, quite often have pathological changes on the oral mucous membrane. Most frequent reaction of the tissues is inflamation of the mucous membrane and fungial infection. Vital role in predispositions of occurence of the prosthetic stomatopathy is played by co-existing disorders and also farmacotherapy. But both, denture and oral hygiene can influence that as well. Chlorhexidine is a specimen that has a vide range of antibacterial and fungicidal effect and also reduces the dental plaque helping treating periodontitis and oral mucous membrane infections.
Material and methods. Examinations were carried out on 30 patients between 51 and 84 with prosthetic periodontopathy of 1st 2nd 3rd class according to Newtons scale with systemic diseases. During 3 consecutive weekly visits, clinical state of oral mucous membrane was examined and values of parameters of oral and denture hygiene after decolourization of bacterial plaque with basic fuchsin. State of parodontium of the residual teeth was examined and qualified. At the first visit patients underwent a process of specialist cleaning and also was instructed about the proper oral hygiene and the way of useing chlorhexidine specimens (Eludril, Elugel). A photo documentation that was created, allowed to notice and examine the changes on oral mucous membrane and on the surface of dentures before and after useing chlorhexidine specimens. During the research subjective opinion of patient was taken into account.
Results. In patients subjective opinion, negative symptoms (pain, itching) disappeared completely or was reduced in 7 days counting from the beginning of using the specimens. There was obserwed reduction of reddening of the area of denture foundation and dental and denture plaque. Improvement of clinical state was noticed in 1st & 2nd class of changes according to Newton.
Conclusion. Specimens containing chlorhexidine (Eludril and Elugel) can be used to support treatment of 1st & 2nd class (according to Newton) of denture foundation changes in or optionally used to sustain the results of complex fungial infection treatment. Patients who use acrylic dentures, especially with systemic diseases, should refer regularly for control visits to evaluate the state of oral mucouse membrane and introduce proper prophylaxis or treatment.
