Porównanie skuteczności wybranych metod diagnostycznych w wykrywaniu i ocenie stopnia zaawansowania wczesnych zmian próchnicowych na powierzchniach żujących pierwszych zębów trzonowych stałych u dzieci

Summary
Introduction: The application of non-invasive methods for the management of caries requires the earliest possible identifica-tion of the disease. Due to the diagnostic limitations of visual and tactile examination, additional methods such as laser-induced fluorescence (LF) and quantitative light-induced fluorescence (QLF) are recommended for the inspection of fissures of the occlusal surfaces of teeth.
Aim: The comparison of selected diagnostic methods accuracy in the detection of early occlusal carious lesions in first permanent molar teeth in children.
Material and methods: The study was carried out on 30 otherwise healthy children, aged 6-8, in which 118 occlusal surfaces of first permanent molar teeth were examined. Ethics Committee approval was obtained. The condition of the fissures was assessed by visual and tactile method, LF (DIAGNOdent) and QLF. In case of the occurrence of early carious lesion in fissures the preventive resin restoration (PRR) I or II were applied. The sensitivity, specificity and confidence interval of diagnostic methods were calculated.
Results: The agreement of examiners was assessed as good (kappa = 0.706). The sensitivity of visual and tactile examination was 85%, and the specificity 70.83%, whereas the sensitivity of the DIAGNOdent was 70%, and the specificity 100%. The highest, 100% sensitivity and the specificity were for QLF.
Conclusion: The visual and tactile examination does not provide full diagnostic effectiveness. The application of QLF in comparison with LF method shows higher detection accuracy and it is the most sensitive and specific diagnostic method.
