Summary
Introduction. The critical values are defined as results of the laboratory tests indicating a life-threatening condition. Every hospital laboratory should establish critical limits according to opinion of hospital physicians.
Aim. The aim of this study was to establish the limits of critical values of most typical laboratory tests in multi-profile pediatric hospital based on information obtained from physicians on the basis of their experience and clinical expertise.
Material and methods. Physicians from the University Children’s Hospital in Krakow were requested to indicate the lower and upper limit of chosen laboratory tests above or below which they should be immediately informed by the laboratory staff. The questionnaire was distributed to 210 physicians. 71 questionnaires were received back, which represents 33.8% of distributed questionnaires. Survey was anonymous, however obtained data have been classified according to the clinics.
Results. We have found significant differences between the critical limits suggested by physicians from different departments. The differences concerned both the lower and upper critical limits. The suggestions within a reference range or exceeded intravital ranges were excluded. Remaining suggestions were calculated (the mean or median) separately for lower and upper limit of the critical values of every individual laboratory parameter.
Conclusions. Determination of critical values in multi-profile children’s hospital is complicated. Optimal would be to differentiate the system of critical limits according to age and sex of the patient and even his disease.
