Summary
The acid-base balance is often evaluated in all branches of anaesthetic practice. There are, however, different approaches this issue. The traditional approach based on the Henderson and Hasselbach equation is very useful in respiratory disturbances but not in metabolic ones. Therefore, many other parameters, such as buffer base, base excess or standardized base excess have been introduced (Copenhagen approach). Another approach is to compare calculated and measured values of carbohydrated ions as well as to determine the so-called anion gap (Boston approach). The great transatlantic debate on acid base status which approach is more accurate did not reach a compromise.
The modern approach to the acid-base balance, based on the physicochemical theory, involves the strong ions difference and total weak acids.
The objective of this review is to present both the principles of the acid-base status evaluation and to discuss the limitations of the available methods.
