Powikłania tracheostomii przezskórnej metodš Griggsa w okresie jej wdrażania

Summary
Background.Percutaneous dilatational tracheostomy (PDT) techniques became common procedures in adult ITUs, because they are simple, fast, safe and cost-effective. We assessed the development of practical skill with the Griggs PDT, when used by experienced physicians and trainees.
Methods. One hundred twenty seven patients were examined. The elective Griggs PDT was performed at the bedside in 125 patients, divided into 5 equal groups of 25 patients (A-E), placed in sequential order (A-E). All patients were also divided into two groups (I-II), depending on the experience of the operator (I – well-experienced person, and II – trainee). The complication rates were calculated in both groups. Fisher´s exact test was used for statistical analysis.
Results. The rates of complications in the first analysis were: A – 20%, B – 12%, C – 8%, D – 8%, E – 20%. There were no significant differences in the complications rates between groups A to E.
73 PDT´s were performed in group I, and 54 in group II. The complication rates were 15% and 11%, respectively. There was no statistical difference in the number of complications (p=0.052) or in technical problems (p=0.087).
Conclusions. The PDT seems to be easy to learn and can be performed by a trainee in properly selected patients. The complication rate of PDT was found to be independent of the operator and institutional experience.
