Porównanie przydatności premedykacji klonidyną lub midazolamem do operacji usunięcia zaćmy w znieczuleniu okołogałkowym

Summary
Objective.To evaluate the efficacy of premedication with clonidine or midazolam in patients undergoing cataract removal under peribulbar block.
Methods. Patients undergoing cataract removal were divided into two groups of 90 each: group K (premedicated iv with clonidine (0.002 mg/kg) and group M (premedicated iv with midazolam (0.02 mg/kg)). Assessment included measurements of: HR, MAP, SpO2, level of sedation and analgesia.
Results. 180 patients (106 women and 74 men aged 69.2±10.1years) were enrolled in a study. The mean SpO2 values did not differ between the analysed groups. In both groups there was a slight drop in HR; however, mean HR values during operation and in the postoperative period were significantly lower in group K. Mean initial values of MAP did not differ between both groups, but in the time directly preceding the peribulbar block and in subsequent measurement periods the mean values of the above-specified parameters were significantly lower in group K (p<0.01). In group M the performing of peribulbar block entailed stronger pain sensations (p<0.05), although the level of sedation was more intensified (p<0.01) in that moment.
Conclusions. Clonidine is more useful than midazolam in premedication of patients undergoing the operation of cataract removal under local anaesthesia since its administering ensures the patients greater comfort connected with lesser pain sensations during the performing of peribulbar block and it displays a stabilizing effect on the cardiovascular system.
