Uogólniony proces zapalny o wielonarządowej manifestacji u pacjentki z hipercynkemią i wysokim stężeniem kompleksu kalprotektyny w surowicy

Streszczenie
We present the case of a child with the symptoms resembling generalised autoimmunity process, with multiorgan manifestation since the early infancy. Very high concentration of zinc was found in the patient´s serum. The zinc binding protein has been identified as calprotectin (S100A8) / (S100A9 complex).
Zinc concentration in serum and erythrocytes was measured by atomic absorption spectrophotometry assay in Department of Laboratory Diagnostics, Children´s Memorial Health Institute, Warsaw. Zinc concentration is constantly increased, even during clinical remission, between 246 mg/dl and 734 mg/dl, which is several times over the normal range (reference range 70-120 mg/dl). Calprotectin in serum was measured by ELISA assay in Department of Child Health, University of Aberdeen. High concentration of this protein complex is more than 1000-times over the reference range.
Presented disturbances probably play the main role in pathogenesis of clinical picture in patient. The cause of the increased concentrations of calprotectin are unknown, but are thought to be due to inborn error of calprotectin synthesis or catabolism.
