Sedacja i anestezja do badań metodą rezonansu magnetycznego – doświadczenia własne

Summary
Background. Magnetic resonance imaging (MRI) settings are noisy, cold and claustrophobic, and are therefore poorly tolerated by children and some adult patients. Sedation and anaesthesia may be technically difficult because of limited access to the patient and special requirements for anaesthetic equipment. We present anaesthetic protocols used in 2112 patients undergoing MRI in our institution. Methods. Over five years, anaesthesia or sedation was provided to 626 children of less than 3 years, 1268 children aged 3 -16 years and 218 adults. Children younger than one year received no premedication, and were induced with N2 O and halothane or sevoflurane. Incremental doses of midazolam were given when necessary. Children older than 3 years were premedicated orally with 0.3 - 0.5 mg kg-1 midazolam. Sedation was maintained with fractional doses of intravenous propofol (0.3 - 0.5 mg kg-1). Adult patients received 10 mg midazolam, followed by intravenous propofol.

Results. Fifty-four patients required controlled ventilation due poor general condition. Apart from the three children who required emergency intubation and ventilation, there were no complications. Conclusion. We regard these protocols as safe and effective and recommend them for routine MRI anaesthetic practice.
