Streszczenie
The increase in cancer incidence should bring on the development of local and international cancer programmes. The range and scope of these programmes should cover prophylactics, treatment and issues concerning the best possible quality of life of cancer patients. Among these aspects effective pain treatment remains an important issue, especially in patients with advanced disease. In Poland standardised pain treatment has been introduced in accordance with WHO guidelines, usually referred to as the „analgesic ladder”. The occasional failures of pain treatment are usually observed in cases of neuropathic pain.
One may assume that physicians do not devote enough care to the diagnosis of pain and, as a consequence, do not apply adjuvant methods of pain treatment as often as it could be recommended. For this very want of diagnosis non-pharmacological methods available at pain clinics are also rarely applied. It would be necessary to introduce a form of „common language” – clear to all medical specialists dealing with cancer pain. The WHO guidelines are a good basis for developing such a language, simultaneously allowing for a development of more complete standards of treatment. These latter should contain a description of the methods of treatment and basic drugs and should also present a clear listing of competencies assigned to oncologists, general practitioners and palliative care and pain specialists, including indications for specialist interventions.
