Summary
Remifentanil, a potent, ultra-short acting synthetic opiod, duo to its unique rapid esterase cleavage to inactive metabolites, is independent of organ clearance and therefore is especially attractive in high risk patients. It has been widely used in cardiothoracic anaesthesia, securing stable haemodynamics during surgery and effective pain control in the postoperative period. Although safety and usefulness of remifentanil in cardiac procedures has been proved, it does not seem superior in comparison to other commonly used opoid analgesic drugs. The use of remifentanil in thoracic procedures will probably be limited to short surgical, endoscopic and diagnostic procedures.
