Desfluran a propofol – ocena porównawcza znieczulenia do ginekologicznych zabiegów laparoskopowych

Summary
Background. New, low soluble volatile anaesthetics may present an useful alternative to propofol in fast track anaesthesia and surgery. The purpose of the study was to compare the clinical characteristics of desflurane-based vs. propofol anaesthesia for gynecological laparoscopic surgery. Methods. Fifty-two female ASA I and II patients were randomized to two groups: one group received inhalational anaesthesia with desflurane, the other continuous infusion of propofol. Intraoperative heart rate and arterial blood pressure were recorded. Emergence was assessed based on the time from cessation of anaesthetic administration to eye opening; the time to recovery of spontaneous breathing; the time to extubation and modified Aldrete score. PONV episodes were noted during the first 24 hours after anaesthesia. Results. Arterial blood pressure during anaesthesia, times: to eye opening, to recovery of spontaneous breathing, to extubation, and modified Aldrete scores, did not differ between the groups. Heart rate was significantly (p <0.05) lower in the propofol group immediately after induction and at 5 minutes after start of surgery. The number of the PONV episodes was also significantly lower in the propofol group. Conclusion. Both agents provided similar clinical anaesthesia and can be safely used for short gynecological surgery.
