Summary
The methods are discussed which are used most frequently for improving the status of arteries of lower extremities in patients with intermittent claudication, that is physical exercises and pharmacotherapy. Correctly performed exercises can prolong the distance of walking without claudication by 50 to 200%, but owing to coexistent diseases only 25% to 50% of patients are able to complete the whole programme of exercises. The aim of pharmacological treatment is to improve microcirculation and to develop collaterall circulation. For that reason vasodilating drugs, antiplatelet drugs and preparations stimulating angiogenesis are used. Besides that, drugs with multidirectional action mechanism are applied, such as prostaglandins, pentifylline, inhibitors of type III phosphodiesterase. Their usefulness is still a subject of multicentre studies and follow-up observations.
