Pyoderma gangraenosum – prezentacja 2 przypadków

Streszczenie
Pyoderma gangrenosum is a disorder of unclear etiology and it is often associated with various systemic diseases. Although it has been known for over 70 years it still remains a diagnostic ant therapeutic problem. It usually appears between the ages of 25 and 54 but it can also develop in childhood and then it coexists with systemic disease. The choice of a treatment method of pyoderma gangrenosum depends on the clinical forms of this disease, the lenght of duration, the intensification of pathological changes and coexisting diseases. A very important element is an adjuvant treatment which combines the care of ulceration, the control of pain and the symptoms connected with pyoderma ganrenosum e.g. a general emaciation of an organism. In this paper we present two cases of pyoderma gangrenosum and we emphasize beneficial results obtained by our patiens by means of sulphosalazine and cyclosporine A combined with medium doses of corticosteroids. In both cases a coexisting systemic disorder was not discovered and it surely facilitated and hastened a good therapeutic result. The diagnosis of pyoderma gangrenosum in these cases was based on the clinical qualities typical of this disease entity- a relatively violent development of ulceration of a typical clinical picture and a good therapeutic result after the administration of corticosteroids. However in many cases a correct and final diagnosis as well as optimum and effective treatment can cause many problems and require a partcular care.
