Przypadek grzybicy woszczynowej skóry gładkiej w przebiegu kortykoterapii

Streszczenie
A case of a 24-year old female patient suffering from tinea favosa of the glabrous skin has been presented. This type of the skin mycosis is nowadays very rarely observed in the European countries, including Poland. Clinical manifestation of tinea favosa is not uniform but characterized by variety in morphological features and lesional localization. Since infancy our patient has been treated for ichthyosis vulgaris and for the last two years the corticosteroids have been additionally administered, both locally and systematically due to atopic dermatitis. Coexistence of these two diseases initially caused difficulty in establishing a correct diagnosis. Non-typical skin lesions have acquired form of tinea incognito. A term of tinea incognito means the dermatophytosis of the altered clinical appearence bleared by using topical and systemic corticosteroids due to misdiagnosis or coexistence of another skin disease. Since corticosteroides can inhibit the inflammatory process, pruritus and suppress the cell - mediated immune response, the mycotic nature of the disease becomes difficult to recognition. The final diagnosis of tinea favosa has been based on the results of the direct microscopy, Wood´s light examination, microscopic and macroscopic evaluations of the funigal culture. Treatment with terbinafine has been administered and has resulted in a complete recovery of a normal skin condition.
